Documents to bring 

--Newlyweds: Bring in a certified marriage certificate and the spouse's Social Security card
--Newborns: Bring in the baby's birth certificate and Social Security card, if you have not received the baby's Social Security number, the military member has 90 days to bring it in
--Recently Divorced: The sponsor needs to bring in the divorce decree and spouse's ID card and any other IDs that need to be confiscated

Dependent IDs

In order for a dependent to get an ID, the sponsor must be with the dependent. If the sponsor is deployed, TDY, etc., the dependent needs to have a Power of Attorney or an original DD Form 1172 signed by the sponsor. If the dependent does not have any of the documentation listed, he or she can receive a temporary ID for 90 days.

Dependents turning 21

If the sponsor' s dependent child is turning 21, the child cannot have an ID unless he or she is a full time student in college. To keep them enrolled in DEERS, they need to produce a letter from the school registrar stating that they are going to school full time. It should also include the date he or she started college and his or her expected graduation date. It needs to be certified, on a school letterhead, and it must be signed by the registrar.

Dependents/Retired members turning 65 and receiving Medicare

Bring in your Medicare card so that it can be added into DEERS. The earliest Medicare can be updated is two weeks before your birth month.

Contractors

Bring in your old CAC if you were previously issued one and another form of ID. If you are a new-hire, bring in two forms of ID and a completed DD Form 2842.

Civil Service

Bring in your old CAC if you were previously issued one and another form of ID. If you are a new-hire, bring in two forms of ID.

Active Duty Members who are Retiring

If you're an Active Duty military member who is within 120 days of retiring and on terminal leave, you are eligible to receive your retired ID card. You must bring in your retirement orders and your Active-Duty CAC.

Disabled Veterans (DAV)

If you are a DAV and have a letter stating your percentage of disability, bring in that letter and two forms of ID.
 
Reserve Retirees

If you're a Reserve Retiree who is turning 60 and will start receiving retirement pay, bring in your Confirmation Letter of Retirement either on or after your 60th birthday along with your old ID card.
 
Lost ID Card

If you've lost your ID card, immediately report the incident to the Security Forces Law Enforcement Desk. The Desk Sergeant will complete an AF IMT 1168/Statement explaining when and where you believe you lost your ID card and what actions you've taken to locate it. The Desk Sergeant will provide a copy of the statement to you.

If you're military, give this to your First Sergeant. He or she will complete a Lost ID Card Letter. Bring this letter to the Customer Support Element for issuance of a new ID card.

If you're a civilian, bring a copy of the statement directly to the Customer Support Element for issuance of a new ID card.
